Applications for Registration and Certification

Applications are for direct accreditation with the Wisconsin Department of Natural Resources (DNR).

The DNR is not a National Environmental Laboratory Accreditation Program (NELAP) accrediting authority. The DNR does not recognize NELAP accreditations issued by other state or federal agencies in lieu of accreditation under Chapter NR 149.

There are no current reciprocity agreements with any state.

The DNR does not offer accreditation for air testing.

The DNR does not offer accreditation for microbiological testing.

The DNR approves laboratories for performing asbestos and radiological tests but does not directly accredit them.

The accreditation process consists of submitting a signed application form 4800-002, application attachment forms (9400-568, B, A1, A2, and A3, as applicable), a check for the application fees, required successful PT results, and any additional required application materials.

Once the application is complete an evaluation will be scheduled unless the application is for transfer of ownership.

Application Types

There are three different types of applications:

  • initial — labs that are not currently accredited in Wisconsin
  • revised — labs that are currently accredited in Wisconsin that wish to change their scope
  • transfer of ownership — labs that change ownership and their FEIN changes

Application forms

  • Form 4800-002, the main application form. This is a dynamic form. Save the form to your desktop and open it up using Adobe. If opened directly in a web browser Microsoft Edge must be used.
  • Form 9400-568, the FEIN Request Form (new applications and transfer-of-ownership applications only)
  • Complete one or more of the matrix-specific analyte attachments listed below.
    • Attachment B - designed for small industrial & wastewater labs performing testing limited to ammonia, BOD, cBOD, chloride, TKN, TP, TSS, and TVS.
    • Attachment A1 - Aqueous matrix analyte offerings.
    • Attachment A2 – non-aqueous matrix analyte offerings.
    • Attachment A3 – drinking water matrix analyte offerings.

Application Cost

The application cost is the sum of Parts 3 through 7 of application form 4800-002. Because WI accredits by technology, determining the correct application costs can be confusing. It is highly recommended that a completed form 4800-002 is sent to Tom Trainor for review before finalizing it or obtaining a check for the fees. Doing this can avoid re-work that stems from over-paying or under-paying for fees.

Proficiency testing (PT) related to application submittals

Laboratories are required to successfully analyze, and report required PTs to the DNR with their application.

  • Application PTs must be no more than six months old from the application received date.
  • The PT study date is equal to the PT study close date.
  • "WS" PTs are only acceptable for Drinking Water matrix.
  • "WP" PTs are only acceptable for Aqueous and Non-aqueous matrices.
  • PTs must be analyzed and reported using an approved method.
  • The method reported for your PT samples must match the analytical technology.

Laboratories applying for initial accreditation must:

  • submit a copy of their quality manual
  • submit form 9400-568 – FEIN request form
  • submit a “letter of intent” to work in WI for laboratories not physically in WI
  • Initial demonstration of capability studies for at least one analyst for each test.
  • MDL studies for each analyte and method for each matrix.
  • SOPs, preparation and analysis, for each method applying for.
  • WP PT results for each matrix-technology-analyte combination that requires PTs.
  • WS PT results for drinking water-method-analyte combination.
  • Fees payable to Wisconsin Department of Natural Resources.
